Transition Services

Transition services help students with disabilities prepare for life after secondary school through a set of coordinated activities that are:

  • Focused on improving the student’s academic and functional achievement to support their transition from school to post-school activities. These activities may include post-secondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, and community participation.
  • Based on the individual student’s needs, taking into account their strengths, preferences, and interests.
  • Inclusive of instruction, related services, community experiences, the development of employment and other post-school adult living objectives, and, when appropriate, the acquisition of daily living skills and functional vocational evaluation.
